On this page, we will let you know about the latest development at the clinic: new technologies, continuing education courses, vacation time.

In the future, we hope to offer you an online reference system. At the moment, we use referral slips which you would give to the patient or mail to us directly. Please call our office for new pads of referral slips if you need more.

On our referral slips, we created a code that will allow us to better schedule your patients in the goal to diminish their waiting time. The code is based on the table in the center of the referral slip.

Here is how to fill our referral slips:

On the left side of the table are the various requests for treatment. Please circle the letter corresponding to the requested treatment.
On the right side of the table is a list of possible descriptions present in the tooth/teeth to be treated. Make a check mark in the circle corresponding to any description applicable to the case.
After all the checks have been made to describe the case, please circle only the highest number with a check mark on the right side.
Example codes may be A3 or C2. The code simplifies the information for the patient and communicates to us all the information we need to schedule an appropriate amount of time for their case. If there are multiple teeth, consider placing the code numbers in the remarks section of the referral slip.
